If you are a New patient who is interested in scheduling with our practice please call our office at (208) 287 - 5525. We will enter your demographics and confirm that we are an in network provider, if you are looking to be seen medically. Whether or not you are a medical or cosmetic patient we need you to fill out online patient paperwork which we will grant you access to over the phone, once paperwork is completed we can place you in the schedule.
Once you complete the online paperwork Every NEW patient is expected to arrive 10 minutes early to their scheduled appointment. This gives the staff time to collect a copy of your ID and insurance card before your appointment.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this web site. Also, take some time to review our staff page and familiarize yourself with the doctor. We ask that you do not bring small children to your appointment. If you are unable we ask that you bring adequate supervision to wait with them in the lobby. This is to ensure that you receive the highest standard of care. We look forward to your first visit.

We are not in network with every insurance company. Please call the office to ensure that we are in network with your insurance company and specific plan type. If we are not in network, and you want to be seen by an in network provider please call the number on the back of your insurance card and they will give you an in network recommendation.
